Optum
EAP
See a comprehensive overview of the pros and cons of the Optum EAP for UK and global employers.
Optum EAP overview
Optum is the health services arm of UnitedHealth Group, and its EAP heritage in the UK is PPC Worldwide, acquired in 2009 and rebranded Optum in 2014. Globally the EAP claims 3,500+ client organisations and around 10 million covered members across 150+ countries, which makes it a default shortlist entry for multinationals wanting one worldwide contract.
In the UK it mainly serves large corporates and financial services employers from its London base. Its UK footprint has been shrinking: UnitedHealth sold its Optum UK health-technology business (including EMIS) to TPG in March 2026.
The public reputation problem is severe: optum.com sits at 1.3 on Trustpilot. Most reviews concern its US care and billing businesses rather than the EAP, but it is what your employees will find when they search the provider's name.

Counselling on Optum
Access is via a 24/7 freephone line, the liveandworkwell.com portal or the Optum Assist app. Callers receive in-the-moment telephone support from counsellors, then referral into short-term structured counselling with a network provider; session counts are set per employer plan, commonly around 6 to 8 in UK-style contracts.
Delivery runs through a large affiliate network of local counsellors rather than an employed clinical team, which enables global coverage but makes local quality variable.
There is no published UK waiting-time data and UK user reviews are almost non-existent, so employers are largely buying on account management rather than public evidence.

Integrations
The EAP integrates with Optum's global wellbeing stack, behavioural health networks and, for US-linked employers, UnitedHealthcare insurance plans. Digital self-help content and wellbeing assessments sit on the portal.
Confidentiality
Standard EAP practice with anonymised employer reporting.
Extras
Critical incident support, manager consultation lines, training and webinars, and work-life referrals covering legal, financial, childcare and eldercare matters.

Best reviews for Optum
Global one-stop coverage
The EAP spans 150+ countries under a single contract, which UK multinationals value for consistency, and three decades of EAP heritage bring deep critical-incident experience.
Established digital front door
The liveandworkwell and Optum Assist platforms bundle counselling access, self-help content and work-life resources, and employees can start by app, web or phone.
Bad reviews for Optum
Dire consumer reputation
The 1.3 Trustpilot profile describes unreachable phone support, endless transfers and unresolved complaints. It is mostly US healthcare customers, but it shapes employee trust here too.
Impersonal at scale, thin UK footprint
Support runs through call centres and an affiliate network rather than named local clinicians, and UK-specific feedback is almost absent. The March 2026 sale of Optum UK's technology arm underlines the shrinking UK commitment.
In Summary
Optum makes sense in exactly one scenario: you run a workforce across 30 countries and need one EAP contract, one data standard and one incident-response process everywhere.
For a UK-first employer, the case is weak. The service is an affiliate-network phone EAP with no published UK evidence of speed or quality, wrapped in a parent brand whose public reputation is at rock bottom and whose UK investment is visibly retreating.
The 2024 Change Healthcare ransomware breach (affecting roughly 190 million people) and ongoing US scrutiny of UnitedHealth add reputational risk that HR teams should at least be aware of when putting the logo in front of staff.
How Optum compares to Spill
Optum offers the same capped phone-and-referral EAP in 150 countries. Spill offers something deeper in one market: unlimited therapy with BACP-registered counsellors from the top 5 percent of applicants, booked by employees themselves in one click from Slack or MS Teams, with same-day availability 7am to 9:30pm, 7 days a week, in 15+ languages.
Spill's pricing is public and starts at £75 a month on a rolling 30 day contract with no exit fees, live within hours. Optum is quote-based, multi-year, and UK buyers inherit whatever affiliate coverage exists in their area.
Choose Optum if you genuinely need one EAP vendor across dozens of countries. Choose Spill if your team is UK-based and you want unlimited same-day therapy people use, not a logo that covers every timezone.
